Nonsurgical Technique Can Take Years Off the Appearance of Aging Eyes

Volumizing the Brow with Hyaluronic Acid Fillers
New York, NY (Vocus) August 17, 2009 — As one ages, the characteristics of the eyes change in generally predictable ways: the skin thins and becomes less elastic, brows may descend, the upper lid may recede or enlarge, and the bones around the eyes appear more prominent. While surgery has long been the accepted standard and offers significant benefits to many patients, injectable treatments are proving to be a viable option for taking years off the appearance of aging eyes. An article appearing in the current issue of the Aesthetic Surgery Journal, a publication of the American Society for Aesthetic Plastic Surgery (ASAPS), details the use of hyaluronic acid (HA) fillers as a reliable and long-lasting method for improving the eye area.

“In some cases, the addition of volume may provide a better-looking result than traditional surgical procedures,” says Val Lambros, MD, author of the article and a plastic surgeon practicing in Newport Beach, California.

“Although the upper lid can be improved with injections, the area we treat is the brow. In some cases, filling in the brow expands the lid skin sufficiently to smooth wrinkles.”
According to Dr. Lambros, adding volume in the brow area can make the eye appear longer and fuller with less upper lid showing, all characteristics of a youthful eye. However, as in all cosmetic procedures, there is no “one size fits all” approach. “These are true aesthetic choices that must be decided through the patient’s consultation with a board-certified plastic surgeon,” advises Dr. Lambros.

Since the 1980s, the standard method for volumizing the brow line has been by fat injection. Excellent results
can be achieved using this method. The risk with fat injections, however, is that over time some of the fat may not survive. In some cases, the injected fat may actually grow as the patient ages or gains weight, resulting in unevenness in the brow area.
Hyaluronic acid has been used to treat the brow and upper lid since its introduction in 2005, using a technique similar to that of fat injections. Dr. Lambros prefers HA to fat for brow volumizing because he says contouring is easier, and HA is less subject to variability than biologic fillers. In the brow area, HA has been shown to produce long-lasting results, extending beyond two years. In addition, the effects of HA can be reversed immediately, using hyaluronidase (an enzyme that degrades hyaluronic acid), should the patient be displeased with the volumization effect.

When it comes to the eyes, a little can go a long way. “Brow volume procedures should not be overdone; more is not always better,” adds Dr. Lambros. “Most candidates for brow improvement with HA elect to begin treatment with small amounts of product, and are very satisfied with the results.”

Dr. Lambros gives patients the opportunity to preview their final look, a benefit not available to patients undergoing surgery. He creates temporary volume by injecting diluted local anesthetic which is threaded into the brow and molded into shape. If desired, the patient can choose to undergo the procedure immediately following the preview, since the brow is already anesthetized.

The use of injectable fillers in the brow is a complicated process and requires great skill and experience. “Because of its dependency on technique, only qualified injectors and board-certified plastic surgeons should be performing this procedure,” states Renato Saltz, MD, president of the Aesthetic Society. “When performed properly and by appropriate medical professionals, the use of HA fillers is safe, effective, and produces
long-lasting results.”

After any surgery, there will a period of time when there is increased swelling followed by recovery and resolution of the swelling. Typically, the swelling increases for the first 4-5 days and peaks out about day 5. Then there is a relatively rapid decrease in swelling for about 6 weeks. At this time usually about 80% of the swelling has resolved. The remaining 20% may take several more months to resolve entirely.

Immediately after breast augmentation there is also some tightness about the implant as these tissues are draped over the implants. As time goes on the tissues will slowly stretch and accommodate to the implant. This entire process may take up to 4-6 months. During this time there is continued softening of the breasts and relaxation of the implant within its pocket.

About a month after surgery, although the entire healing process is not yet complete, the breasts begin to have a very pretty appearance since enough of the swelling has resolved and some softening has developed. Nevertheless, the final result will not be apparent until about 6 months.

Finally, after having done breast augmentations for more than a decade, my experience is that, with rare exception, most women after about 6 months wish they had chosen an implant somewhat larger than what they actually did choose. My recommendation to you is that you be patient for the time being, allow things to heal and settle down before you make a decision about changing size. In the big scheme of things you are still relatively early in the healing period.

Breast Augmentation – Five Things You Need to Know before Surgery

1. Size matters.
Obviously any woman desiring a breast augmentation wants to be fuller and larger and this can be readily accomplished with a large variety of implant sizes and/or types. Women who have had previous pregnancies often have some breast skin laxity. In order to get an optimal result, it may be advantageous to choose an implant large enough to fill out the breast and restore the projection and “perkiness” of the breast. This will also fill out clothing and add to the feminine figure. Nevertheless, the initial enthusiasm for choosing a large implant should be weighed against the potential long term disadvantages of supporting the weight of these implants. Large heavy implants can over time lead to stretch of the skin and excessive dropping of the implant. It is difficult to predict exactly what size will lead to this situation and the recommendation of the surgeon should be carefully considered.

2. Incision doesn’t matter.
The three most common locations for the incision are in the skin crease at the bottom of the breast, around the edge of the nipple and in the armpit. An excellent cosmetic result can be achieved with any of these incisions. The recovery, scarring and risks are all essentially the same. Some surgeons (including myself) may have a personal preference but the final result should be independent of which incision is used.

3. Implants are different.
Breast implants are filled with either saline (a salt water solution such as IV fluid) or silicone. Both are good implants and excellent results can be achieved with either type of implant. Nevertheless, there are some differences. The silicone implants feel softer and more realistic. Further, after augmentation the tissues of the breast tend to drape over the silicone implants a in a more natural fashion and give a prettier appearance. After years of study, the FDA has determined that there are no health hazards associated with silicone filled implants as compared to saline filled breast implants. They are available to any woman 22 years old or older.

4. Placement can be either above or below the chest wall muscle.
The pectoralis major muscle can be used to add additional padding or coverage over the implant during breast augmentation. This is especially advantageous when using saline filled implants, which have a tendency to ripple. The rippling can cause the appearance of waves on the surface of the breast especially in the upper portion and especially if there is relatively little breast tissue covering the implant. Placing the implant under the muscle minimizes this risk.

5. Surgeon choice matters.
Surgeons certified by the American Board of Plastic Surgery (ABPS) have undergone extensive training and testing under the guidelines of the ABPS specifically related to plastic surgery including cosmetic surgery. Plastic surgeons who are members of the American Society for Aesthetic Plastic Surgery (ASAPS) have also demonstrated particular interest in cosmetic surgery and are required to maintain levels of continuing education. In addition to credentials, you must feel comfortable with your choice of surgeon and also realize that this is your operation. Your surgeon should give you information and advice about the procedure, but in the end the decision should be yours. You should be able to choose your own implant size, style, location of incision and so forth.

I have abdominal separation and umibilical hernia, but I am more concerned about baggy skin on my stomach. I want it removed without getting my muscles fixed to decrease my down time. Is this possible? Would it be worth it?

Tummy Tuck (Abdominoplasty) is an operation designed to improve the contour and shape of the abdomen. Often pregnancy or significant weight gain can stretch not only the skin of the abdomen but also the muscle layer as well. A typical abdominoplasty not only removes the excess skin but also tightens the abdominal wall in those cases when it is necessary. Commonly, we perform some limited liposuction of the abdomen along with the abdominoplasty to further improve the contour and ensure that there is smooth transition from the abdominal flap that has been brought down to the pubic area along the incision line.

Muscle tightening is sometimes not necessary if there has been no significant muscle stretch and the patient has exercised and redeveloped good abdominal muscle tone. Unfortunately, in many cases there has been some stretch of the linea alba. This is a strip of nonmuscle containing tissue that runs down the midline from the bottom of the breastbone to the top of the pubic bone. It runs between the two rectus muscles that comprise the “six-pack” seen on body builders. Similarly to skin, there is no amount of exercise that will tighten the linea alba. Thus the only choice here is to surgically tighten (plicate) this layer during an abdominoplasty.

Tightening of the abdominal muscle layer also allows the surgeon to correct any abdominal wall defects such a hernia. This is a valuable procedure in that hernias may enlarge with time and more importantly a hernia may lead to a serious surgical (and possibly life threatening) emergency if the intestines become trapped in the hernia sac.

I would recommend that you go ahead and have the muscle layer repaired in the situation described here. First, the hernia should be addressed for the reasons cited above. Secondly, skipping this step will significantly reduce the quality of the result that you will get from a tummy tuck. Muscle tightening will flatten the stomach giving a very aesthetic result. Tightening the muscle layer offers no significant increase in surgical risk or complications and you still will need to recover from the surgery with or without muscle tightening. I wouldn’t let the modest increase in recovery from the muscle tightening sway me away from having it done. You’ll get a much better result and I think that in the long run you’ll be much happier with your surgery instead of opting for a half result.
